Error reporting in organizations
Article Abstract:
Human error is at the root of many organizational problems including airline crashes and hospital deaths and error reporting is often the only means of alerting organizations to errors and the circumstances leading to them. A framework of individual error reporting that draws from research on human error, learning, discretionary behaviors, and high-reliability orgainizations is developed, and its theoretical implications and directions for future research discussed.
