Paedophilia and hyperprolactinaemia

The hypothalamus secretes hormones that control the secretion of the anterior pituitary hormones which include prolactin and luteinizing hormone (LH). Men with raised prolactin levels (hyperprolactinemia) have been shown to have a high incidence of sexual problems, including loss of drive and erectile and ejaculatory dysfunctions. Abnormalities of the hypothalamo-pituitary-gonadal axis (HPGA) have also been implicated in some sexually deviant behaviors. In one study, pedophiliacs (child molesters) were shown to have an elevated LH response to luteinizing-hormone releasing hormone (LHRH) compared with non-pedophile sexual deviates and normal volunteers. The case report of a 29-year-old store clerk who was referred to the courts for sexually abusing a six-year-old girl is presented. An endocrine assay revealed that he had unusually high prolactin levels and a low testosterone level. He was sentenced to three years probation with medical treatment. Treatment with bromocriptine returned his prolactin levels to normal within six weeks. Over the three-year follow-up period, prolactin levels remained normal and the patient reported a return of normal sexual drive and sexual function. He also claimed that his sexual fantasies only concerned adult women, but it is not known whether this claim was influenced by the fact that he was on probation. It is suggested that pedophilia may develop, in part, as a result of hormone-related sexual dysfunctions that preclude the ability to maintain adult sexual relationships. Since HPGA dysfunction, sexual problems, and pedophilia seem to be linked, endocrine screening should be considered when evaluating this type of sexual deviance.
