Cholesterol: myth vs reality in pediatric practice
Article Abstract:
Steps to prevent heart disease in adulthood should be initiated in childhood. Children generally fall into two categories of risk. Those at high risk, that is those whose parents developed heart disease early in life, require individualized care. For those at average risk, the goal should be to lower dietary intake of saturated fat and cholesterol and to prevent smoking, maintain weight and increase exercise. However, a 1993 study on the efficacy of dietary recommendations and counseling for parents whose children have high blood lipid levels found that levels of low-density lipoproteins fell in only 19% of the children. Another study on cholesterol screening of children in a pre-paid health program found that noncompliance significantly limits the effectiveness of programs designed to lower children's cholesterol levels and thus their risk of heart disease.

Do primary care physicians prefer dictated or computer-generated discharge summaries?
Article Abstract:
Primary care physicians may prefer computer-generated discharge summaries (CDS) to dictated discharge summaries (DDS) from a newborn intensive care nursery. Among 45 pediatricians and family physicians who responded to a survey, 73% preferred the CDS, 18% had no preference and 9% preferred the DDS. Primary care physicians either read or scan discharge summaries for information on a patient's hospital treatment, current status and continued care needs. Advantages of a CDS include lower cost, immediate availability and consistent format. A disadvantage is less description of the patient's hospital course and problem-solving strategies used in treatment. Most physicians preferred the CDS regardless of their time in practice, type of practice, use of computers in daily life or preference for reading or scanning summaries.
