Nurses should not hesitate to opt for 'aggressive care' methods when dealing with terminally and seriously ill patients. Recognising their essential role in providing a full range of health care to patients, they should have the confidence to strive to create the best possible environment for a dying patient. In this case, even the most basic functions should be performed by a fully trained nurse, not left to care assistants or students.

The DOMINO effect
Article Abstract:
DOMiciliary IN-OUT (DOMINO) is a community antenatal care scheme which is handled by the patient's General practitioner and local community midwife. When the time for delivery comes, the patient attends a local general hospital where she is cared for by the local community midwife, and usually returns home within six to 10 hours of delivery. All postnatal care is handled by the community team. Information on a case history is included.

Nurses should object to non-disclosure clauses in Health Authorities' employment contracts. The code of professional conduct already ensures confidentiality for patients. Open discussion on standards would improve care quality and increase patients' faith in the health care system. Practitioners' willingness to complain about inadequate care is essential as resources are being eroded.
