B-cell lymphoproliferative disorders after bone marrow transplant: an analysis of ten cases with emphasis on Epstein-Barr virus detection by in situ hybridization

Epstein-Barr virus (EBV) is a ubiquitous herpes virus that can cause the activation and proliferation of B cells. In healthy individuals, B cells infected with EBV are promptly brought under control by the immune system. However, in immunocompromized individuals, a variety of disorders may ensue, including lymphoproliferative disorders and potentially fatal infectious mononucleosis. The authors report the cases of 10 patients who suffered a B cell lymphoproliferative disorder (BLPD) after receiving bone marrow transplantation. The symptoms of BLPD developed after widely ranging intervals, from 30 days to over 4 years. Most cases, however, developed within three months of bone marrow transplantation. The techniques of molecular biology were used to examine tissue specimens for the presence of Epstein-Barr virus. EBV genes were found in specimens from all 10 patients, but it should be emphasized that not all abnormal cells were found to contain EBV genes. It is not known if this represents a limitation in the sensitivity of the detection method, or more fundamental aspects of the lymphoproliferative disorder itself. Nine of the 10 patients in the present study died. The sole survivor was treated with interferon-alpha and intravenous immunoglobulin therapy. This case lends support to published reports suggesting that this therapy might prove useful in the treatment of lymphoproliferative disorders in patients with either primary or secondary immunodeficiency.
