Clinical assessment algorithm: subacute care admission decisions
Article Abstract:
Subacute care providers should develop an algorithm to ensure that patients are admitted at the proper level of care required. Each level has a different per diem rate and provides different levels of service depending on patient need. The algorithm should include the patient's age, physical and mental status, diagnosis, medications, need for specialized equipment or outpatient therapy, predicted length of stay, and need for physician input.
