Client assessment in practice
Article Abstract:
Nurses' assessment of their patients or clients, and patients' assessment of them have possible effects on the care and condition of patients. Nurses often make generalisations about patients based on their experience of others and subconscious knowledge. The assessment process is based on comprehending the filters through which patients perceive the world. Evaluation of an early assessment in the light of later care and knowledge shows how accurate the initial assessment was. Case studies of six patients are also presented.

Despite research into the theory of wound management, assessment still poses a problem. Accurate assessment is important for planning appropriate care. Nurses should be knowledgable about the healing process, and be able to recognise tissue types and signs of infection. Assessment must be objective.
