Depth of endometrial penetration in adenomyosis helps determine outcome of rollerball ablation
Article Abstract:
A biopsy of the uterine muscle taken to determine the depth of invasion of uterine lining into muscle may be used to determine whether patients should undergo electrode ablation of the uterine cavity or hysterectomy. Researchers obtained uterine muscle biopsies from 50 women who underwent uterine ablation for excessive menstrual bleeding. Minimal invasion of the uterine lining into the muscle layer was effectively treated by ablation. Deep invasion into the muscle layer was not effectively treated with ablation; hysterectomy would be a better treatment. Women were followed up for over three years.

Partial rollerball endometrial ablation: a modification of total ablation to treat menorrhagia without causing complications from intrauterine adhesions
Article Abstract:
Partial endometrial ablation may be more effective in treating excessive bleeding. Normally, surgeons remove the entire uterine lining, but this can cause excessive scarring. In a study of 50 patients, surgeons only removed the lining on one side of the uterus. None of the women developed adhesions.

An enzyme called matrix metalloproteinase-14 may be responsible for the migration of endometrial stromal cells into uterine muscle tissue. Normally, the endometrium, or inner layer of the uterus, is separated from the muscle tissue by a layer of smooth muscle cells and extracellular matrix.
