Don't deify the role of gatekeeper
Article Abstract:
Lower-paid general practice physicians and nurse practitioners are receiving positive attention as 'gatekeepers,' medical personnel who can treat many health problems and reduce reliance on costly specialists. However, while they may control costs, this may be by under-treating patients. In addition, there is a movement to drastically curtail the number of specialists who graduate from medical schools and increase the number of primary care physicians. This may seriously limit the number of specialists available in many communities and result in a shortage of teaching personnel in medical schools.

Healthcare providers should use care in selecting physicians for integrated groups. Integration involves matching physicians with shared objectives and balancing the specialties and geographic distribution of physicians to the needs of the patient base. Peer reviews and accountability are also important for the long-term success of healthcare organizations.

Article Abstract:
The medical community is being forced to bypass the emotional and subjective aspects of medical treatment to meet the scientific goals of medicine. Physician authority is often undermined by the drive to lower health care costs. The proliferation of managed care plans will continue to shape the delivery of health care, both positively and negatively.
