A comparison between visual and computer analysis of antepartum fetal heart rate tracings
Article Abstract:
Intraobserver and interobserver agreement in the visual assessment of fetal heart rate (FHR) tracings appears to be low. Five expert observers analyzed FHR recordings that were good quality and at least 30 minutes' duration. They were asked judgment and accuracy-related questions such as: What is the baseline FHR? Is the baseline FHR high, normal, or low? Should FHR monitoring continue? Their interpretations were then compared with computerized analyses of the FHR tracings. Interobserver agreement was lower than intraobserver agreement and poor for judgment questions like whether the tracings were abnormal. The observers accurately detected the absence of FHR decelerations but failed to detect computer-detected decelerations more than 90% of the time. In their second and third assessments of FHR acceleration, observers agreed with their first assessment 86.4% of the time. However, observers detected only 65% of the computer-detected FHR accelerations.

Fetal placental embolization in the late-gestation ovine fetus: alterations in umbilical blood flow and fetal heart rate patterns

Even substantial reductions in blood flow to the fetus may have little effect on the fetal heart rate. Researchers implanted sensors in 18 pregnant ewes and their fetal lambs during late pregnancy. They decreased fetal blood supply and oxygenation by injecting latex into the fetus to block off placental blood vessels. Reducing umbilical blood flow by 15% over a period lasting 10 days had no effect on fetal heart rate. Suddenly reducing blood flow by half consistently caused only a marked decrease in the average number of fetal heart rate accelerations.
