Immunophenotyping in the management of gastric lymphoma

Gastric lymphoma (tumor of lymphoid tissue) is a rare stomach malignancy. This is a non-Hodgkin type of lymphoma, involving sites outside the lymphatic system. When the gastrointestinal system is involved, the stomach is the most frequent location. Gastric lymphomas have a high rate of spread and recurrence, and are treated with surgical resection. Accurate and early diagnosis is important in improving response to treatment, which usually also includes systemic chemotherapy, and may include radiotherapy as well. A report is presented of 17 patients with upper abdominal pain, gastrointestinal bleeding, loss of appetite, weight loss, and an early feeling of fullness after eating. Diagnosis of gastric lymphoma was established by immunophenotyping snap-frozen endoscopic biopsy specimen. This involved analysis, using immunological techniques, of biopsies taken via an endoscope, a technique allowing visualization of an interior cavity of the body using an illuminated optic instrument. Eleven patients underwent resection of the tumor. There were no deaths immediately following surgery. Two patients had only surgical treatment; they are free of disease at 3 and 28 months. Nine patients had both surgery and chemotherapy; average follow-up was 25 months and 57 percent are alive at two years. Six patients had chemotherapy only; two-year survival was 75 percent at average follow-up of 16.2 months. Four patients died from progression of lymphoma; two of these patients had levels of Ki-67, a marker of tumor proliferation, greater than 45 percent. The Ki-67 levels in patients who survived was less than 30 percent. It appears that a lower level of Ki-67 is associated with a better outcome in patients with gastric lymphoma.
