Nonsteroidal anti-inflammatory drugs and renal disease - still unsettled
Article Abstract:
A number of case reports have linked nonsteroidal anti-inflammatory drugs (NSAIDs) to kidney failure. The association between NSAIDs and acute and reversible kidney failure has been accepted for several years, but few studies have shown definitive evidence that these drugs can cause chronic or persistent and irreversible kidney failure. One recent report seems indicates that NSAIDs are indeed capable of causing chronic kidney failure. That study's epidemiologic methods were good, but the patients studied may not be representative of the general population. The subjects in the study were all from a referral center in a teaching hospital, not from the community at large. The study only showed that chronic renal failure was more common in elderly men who used NSAIDs for a year or more. Despite these limitations, the study offers enough evidence for a role of NSAIDs in certain cases of chronic renal failure to warrant more research. Case managers, working with treating physicians, can closely monitor patients' lifestyles, diet, and drug treatment while they work to achieve glycemic control. Nurse case managers typically practice under the guidance of clinical treatment protocols, ensuring that patients receive appropriate and correct care. Case managers may also improve the quality of patient visits and encourage patients to effectively manage their disease.

Guidelines for planning a comprehensive at-home management plan for patients with long-term illnesses are presented. Essential plan elements include identifying special needs, setting goals, itemizing a course of action, organizing support and educational services, and providing routine follow-up. These elements are appropriate for a wide variety of illnesses and can be carried out through the combined efforts of health care providers, support services, family, and the patients themselves.
