Placental transfer of sulindac and its active sulfide metabolite in humans
Article Abstract:
Sulindac may prove to be a safer alternative to indomethacin in suppressing preterm labor. Both inhibit prostaglandins, a family of compounds involved in the initiation of preterm labor. Nine fetuses had 18 intrauterine transfusions to treat anemia. The ability to obtain a small blood sample from the umbilical artery during the procedure provided an opportunity to test the extent to which sulindac and its active metabolite crossed the placenta. Mothers were given an oral dose of sulindac a few hours before the transfusion procedure. Sulindac passed through the placenta much more readily than its active form. However, until it has undergone conversion to its active form, sulindac is biologically inert. Fetal levels of the active form were low, suggesting that the fetus has little ability to accomplish the conversion itself. Thus, sulindac may permit treatment of preterm labor in the mother without causing the severe adverse effects to fetal kidneys, intestines, heart, and brain seen with indomethacin.

The use of color flow Doppler ultrasonography to diagnose umbilical cord entanglement in monoamniotic twin gestations
Article Abstract:
Color flow Doppler ultrasonography may be an effective method for detecting umbilical cord entanglement in monoamniotic twins. Monoamniotic twins are identical twins that develop within the same amniotic cavity. Color flow Doppler ultrasonography was used to examine the umbilical cords of three sets of monoamniotic twins. Umbilical cord entanglement was detected in all three sets of twins using color flow Doppler ultrasonography. Branching of the umbilical artery was detected with a different fetal heart rate in each of the branches. Compression of the umbilical vein could be detected by increased blood flow velocity. Two of the mothers were admitted to the hospital immediately to monitor any possible complications caused by entanglement of the umbilical cords. Both of these women underwent an urgent cesarean section.
