A case-control study of cocaine use in pregnancy
Article Abstract:
Women testing positive for cocaine at the time of birth were more likely to have certain complications, but low birth weight associated with smoking cigarettes and lack of prenatal care rather than cocaine use. Outcomes were compared between 138 patients with a positive cocaine screen and 276 cocaine-negative patients matched two to one to cocaine-positive patients for race, age, number of children, and month of birth. Cocaine-positive women were more likely to give birth preterm, to have meconium-stained amniotic fluid, to have premature rupture of membranes, vaginal bleeding, or premature separation of the placenta from the uterus. Syphilis, gonorrhea, and hepatitis B were more common as well. Only 45% of cocaine-positive women had prenatal care versus 86% of control women, and 67% of cocaine-positive women smoked cigarettes versus 35% of control women. Babies of mothers who used cocaine had a statistically higher incidence of low birth weight when smoking and lack of prenatal care were factored out.
