Fluoxetine in the treatment of premenstrual dysphoria
Article Abstract:
Fluoxetine, or Prozac, may be effective in treating premenstrual dysphoria, generally known as premenstrual syndrome (PMS). The symptoms of PMS include tension, depression, irritability, and emotional instability. A total of 313 women who suffered from PMS were divided into three groups: the first group received a placebo, the second received a 20 mg/day dose, and the third received 60 mg/day dose. The women took the placebos or drugs for 6 menstrual cycles and rated the severity of their symptoms on a 0 to 100 scale, with 100 being the most severe. In the placebo group, the average symptom severity went from 56.0 before treatment, to 51.1 in the first menstrual cycle. In the 20 mg/day group, this scale went from 57.2 to 32.4. In the 60 mg/day group, the scale went from 56.3 to 26.6. However, 33% of the women in this last group did not complete the trial because of side effects from the drug. Similarly, 26% of the placebo group dropped out because of the persistence of their symptoms. Thus, a dosage of 20 mg/day or less seems to effectively treat PMS symptoms.

Premenstrual syndrome - pathophysiologic considerations
Article Abstract:
Estrogen may be important in the cycle of mood changes, pain and other symptoms of premenstrual syndrome. Women who experience a cycle of symptoms during menstruation have been treated in a number of ways, but the precise physiologic mechanisms in this syndrome are unknown. Researchers who blocked hormone secretion, then replaced estrogen and progesterone in women with the syndrome found that both hormones could produce symptoms. Evidence for a role of estrogen in the manifestation of premenstrual syndrome may suggest a new approach to drug research and therapy.
