Primary pulmonary hypertension in pregnancy: case report
Article Abstract:
Medical management allowed a woman with primary pulmonary hypertension to survive pregnancy and give birth to a healthy infant. Primary pulmonary hypertension is a kind of high blood pressure in the pulmonary arterty and is rare, progressive, and fatal. The added demands and physiologic changes of pregnancy and the early postpartum period lead to a maternal death rate over 50%. A 26-year-old woman had the disease for a year before becoming pregnant. She was first seen at 24 weeks gestation, by which time the an abortion would have been as dangerous as continuing the pregnancy to term. The fetus was growth retarded but otherwise normal. The woman wished to continue the pregnancy. On the basis of some recent data, she was treated with anticoagulants and nifedipine, a calcium-channel blocker. Her condition improved. At 34 weeks, she had congestive symptoms. Labor was induced, but induction failed so a cesarean was performed. The baby weighed 1756 g and mother and child did well.

A new diagnostic method to detect ectopic pregnancy at a very early stage
Article Abstract:
A new diagnostic procedure appears to detect ectopic pregnancy before symptoms occur. An ectopic pregnancy is a gestation located away from the normal position in the uterus, often in the fallopian tubes. Transvaginal hysterosalpinography-contrast ultrasonography has detected ectopic pregnancy in the early stages when the mass is very small and the area is obscured by bowel loops. Transvaginal ultrasonography can detect intrauterine fluid concentrations. The additional cervical injection of a contrast agent before transvaginal ultrasonography allows the clinician to see accumulations that may be an ectopic pregnancy. Some ectopic pregnancies are missed by vaginal ultrasonography. This method, which can be performed in an ambulatory setting, can provide early diagnosis, thus preventing rupture than can lead to fertility loss.

In vitro cytokine and prostaglandin production by amnion cells in the presence of bacteria
Article Abstract:
Laboratory use of cells from the innermost layer of the placenta may be used to determine which types of bacteria may stimulate cervical ripening and preterm labor. Researchers obtained amniotic membranes from two near-term cesarean deliveries and incubated amnion cells with different concentrations of group B streptococci. The incubation caused production of interleukin-6 and -8 to rise significantly in relation to the concentration of bacteria. Production of interleukin-1-beta and tumor necrosis factor-alpha did not increase; subsequently, levels of prostaglandin E2 did not increase. Preterm labor may be precipitated by increases of prostaglandins.
