Emergency peripartum hysterectomy and associated risk factors
Article Abstract:
Cesarean delivery and the resulting complications appear to increase the risk of emergency hysterectomy following childbirth. The uterine scar formed by prior cesarean section increases the risk among pregnant women of uterine rupture, placenta previa, which is a placenta that develops in the lower part of the uterus, and placenta accreta, which occurs when part or all of the placenta adheres directly to the uterine wall. Of 123 women who had emergency hysterectomy following childbirth, all but seven required cesarean delivery. Overall, 82 of the women (66.7%) had previously had at least one cesarean delivery. Of the 58 women with placenta previa, 40 (69%) had previously had at least one cesarean birth. Placenta accreta was the most common risk factor for hysterectomy, and 41 (67.2%) of the 61 women diagnosed with placenta accreta had previously had a cesarean section.

Amnioinfusion in women with previous cesarean births: a preliminary report

The practice of amnioinfusion may be safe for pregnant women who have had previous cesarean deliveries. Amnioinfusion involves infusing saline solution into the uterus during labor to correct lack of fluid or meconium-stained amniotic fluid, or to relieve fetal distress. Researchers reviewed the records of 936 pregnant women who were given amnioinfusion for various reasons, 122 of whom had previous cesarean deliveries. Seventy-one (58.2%) of the women who had had a cesarean before were able to go through labor and deliver vaginally after amnioinfusion. The other 51 (41.8%) women had repeat cesarean deliveries. One of these women experienced a uterine rupture and had an emergency cesarean. Neither she nor her baby were harmed. Previous uterine scars may not be deterrents to amnioinfusion.
