Rapid changes for academic medical centers
Article Abstract:
Many academic medical centers are already planning for the changes they will need to implement to compete in an era of managed health care plans. Academic medical centers are responsible for training all physicians and provide almost half the charity care in the U.S. They may need to downsize, reorganize and operate more efficiently if they want to compete with community hospitals that are offering their services to large health plan purchasers such as employers and governments. Several medical schools such as Stanford and Johns Hopkins have already formed new organizations called clinical service or physician-hospital organizations. These organizations will bid for contracts to provide managed care plans. Many academic medical centers are affiliating themselves with community physicians or buying their practices. These physicians would then refer patients to the medical center for specialized treatment.

The American health care system: Medicaid
Article Abstract:
Medicaid has not been able to achieve the vision of the congressmen who initially created the program. Medicaid was supposed to provide the same health care to the poor as privately insured Americans received. However, most poor people covered by Medicaid receive health care in clinics and public hospitals. In addition, Medicaid payments to doctors and hospitals do not completely cover their costs. And many states have shifted patients into managed care programs. In 1997, 16% of the American population was uninsured, and most are employed. Many of these Americans do not qualify for Medicaid.
