Risk factors for complications of chronic anticoagulation: a multicenter study
Article Abstract:
The risk of bleeding events associated with long-term treatment with warfarin may be decreased by attention to certain risk factors, frequent check-ups during early treatment and careful patient selection. Warfarin is an anticoagulant used to prevent the formation of blood clots in patients with an increased risk of a stroke or heart attack. Among 923 patients undergoing long-term treatment with warfarin who were followed over an average of two years, 1,332 bleeding events occurred. Thirty-one bleeding events were life-threatening, 226 were serious and 1,071 were minor. Risk of a first episode of life-threatening or serious bleeding increased with the number of years of treatment with warfarin. Thirty-two percent of the patients who had a life-threatening or serious bleeding event suffered a recurrence. Certain factors were associated significantly with an increased risk of a bleeding event.

Predicting risk for prolonged high INR after too much anticoagulation
Article Abstract:
Patients who are older, have heart failure or cancer, who already had high INR values, or required smaller doses of warfarin may continue to have high INR values even when warfarin is discontinued. Warfarin is an anticoagulant and a high INR value means the patients has received too much warfarin and has a high risk of bleeding.

Managing excessive warfarin anticoagulation
Article Abstract:
A study published in 2001 found some patients are at risk of being excessively anticoagulated with warfarin. This can increase their risk of bleeding. Such patients should be treated with oral vitamin K to counteract the excessive coagulation.
