Root canal calcification associated with prednisone therapy: a case report

Systemic lupus erythematosus (SLE) is an autoimmune disease that affects the skin, kidneys, central nervous system, and joints. It also causes lesions of the mucous membranes in the mouth. SLE is treated with nonsteroidal anti-inflammatory drugs, but in acute episodes, corticosteroids may be administered. These drugs have many side effects, including accelerated osteoporosis, a condition involving bone loss. Animal studies have shown narrowing of tooth pulp chambers (the cavities in which tooth pulp is located) and tooth root canals after cortisone administration. A case study is reported of an SLE patient who received long-term high-dose prednisone (a steroid hormone). She was initially found to have moderate periodontitis (inflammation of the gums) and cavities. She received instruction regarding oral hygiene and her teeth were treated as indicated. She continued taking prednisone for another five years, then developed several complications of SLE. One year later, she had developed extensive calcification of all pulp chambers and root canals, with no symptoms, however. Other research has reported narrowing of root canals in patients on high doses of prednisone for long periods. Organ transplant recipients are likely to take steroids for long periods as part of an immunosuppressive program, and the effects on their teeth should be considered. Provisionally restoring a necrotic tooth while maintaining root canal access
Article Abstract:
A simple and quick way of restoring a badly broken-down mandibular left canine that allows proper rubber dam isolation while maintaining access to the root canal system is described. A direct resin restoration using an endodontic file and tapered sleeve assembly, which will function as an internal matrix or form, has helped in provisional restoration while maintaining access to the root canal system.
