Evidence of active cytomegalovirus infection in clinically stable HIV-infected individuals with CD4+ lymphocyte counts below 100/microliters of blood: features and relation to risk of subsequent CMV retinitis

HIV-infected people with cytomegalovirus (CMV) in their urine and low CD4+ cell counts appear to have an increased risk of developing CMV retinitis even if there is no indication of virus in their blood. Retinitis is an inflammation of the retina. Researchers tested the blood and urine of 93 HIV-infected people in good health, with antibodies to CMV in their blood, and a CD4+ count of 100 or less. Only two participants had CMV in their blood cultures, but 48 (52%) had CMV in their urine cultures. The likelihood of having CMV in urine was higher among participants with CD4+ counts below 50 than among those with CD4+ counts above 50. Seventy-eight of the participants had an eye exam and one was found to have retinitis. During an average follow-up of 9.1 months, CMV retinitis developed in 13.5% of the participants who had positive CMV urine tests, but in only 1.9% of the participants who had negative CMV urine tests.

Effect of renal disease and hemodialysis on foscarnet pharmacokinetics and dosing recommendations

The dosage of the drug foscarnet must be adjusted in HIV patients with kidney failure. This drug is given to HIV patients to prevent or treat cytomegalovirus infections. Researchers measured the elimination of the drug in blood and urine samples from 26 HIV patients. Some had normal kidney function, some had mild or severe kidney disease and some with severe kidney failure were on dialysis. HIV patients with no kidney disease eliminated the drug much faster than those with kidney disease. Dialysis removed only about 40% of the drug from blood.
