The Wigan Chronic Obstructive Pulmonary Disease (COPD) Acute Assessment Unit offers in-home care to patients with COPD, reducing the need for lengthy hospitals stays. The nurse-led unit offers in-home followup care for two weeks after the patient leaves the hospital and then clinical care leading to a six month reassessment.
