The Carondelet model: case study 1

The Carondelet Health Network located in Tucson and southern Arizona has served as a demonstration model for a nurse-managed care facility since 1992, primarily serving an elderly population. Core components of the Carondelet plan include assessing member risk when enrolling, integrating all levels of disease prevention, and interventions for groups and individuals. The Carondelet model has reduced health risks, hospital admission, and home visits per episode of care in its service area.

Community nursing centers - issues for managed care: case study 4
Article Abstract:
A nursing center was established in an urban community in Milwaukee WI to encourage managed care networks to include community nursing practice. Since 1979, the Silver Spring Community Nursing Center (CNC) has cooperated with social service agencies based in the community. Medical services at the Silver Spring CNC are based on contractually-defined aspects of health care delivery and have been changing as community needs change.

The Carle clinic: case study 3
Article Abstract:
The Carle Clinic, a Community Nursing Organization based in Urbana IL, serves a largely rural, aged population. Coordinated care for this facility is designed by nurses and based on enrollee assessment. Nurses also schedule physician visits, transportation, and meals delivered to the home. Success of this model for managed care depends on developing the appropriate combination, intensity, and length of services.
