Thrombolytic Therapy for Ischemic Stroke: From Clinical Trials to Clinical Practice
Article Abstract:
Doctors need to be careful when using tPA in stroke patients. tPA breaks up blood clots, which are the cause of most strokes. Two studies show that doctors in the community often did not follow guidelines for using tPA. One of the most commonly violated guidelines was giving tPA within a hour or two of the onset of symptoms. Also, in one study stroke patients who received tPA had higher mortality rates than stroke patients who did not receive tPA. Over 20% developed bleeding in the brain, a common side effect of tPA.

Ximelagatran vs warfarin for stroke prevention in patients with nonvalvular atrial fibrillation: A randomized trial
Article Abstract:
The study aimed to compare the efficacy of the oral direct thrombin inhibitor ximelagatran with warfarin for prevention of stroke and systemic embolism. It concluded that the efficacy of fixed-dose oral ximelagatran without coagulation monitoring compared with well-controlled warfarin for prevention of thromboembolism in patients with atrial fibrillation requires chronic anticoagulant therapy, but the potential for hepatotoxicity requires further investigation.

Ensuring Patient Safety in Clinical Trials for Treatment of Acute Stroke
Article Abstract:
Stroke patients who are enrolled in clinical trials of various drugs must be protected from the potential adverse effects of the drugs. One study was stopped after an evaluation of 300 patients showed the patients who received the drug had higher mortality rates than those who received a placebo. But while that analysis was being done, another 328 patients were enrolled in the study.
