Transabdominal cervicoisthmic cerclage: a reappraisal 25 years after its introduction

Cervical cerclage is a surgical technique that is used for treating pregnant women with an abnormal cervix. The procedure involves placing a loop or band around the cervix to hold it in its proper position. In many cases the surgery is performed transvaginally (through the vagina). However, in cases where the cervix is too short, deformed, scarred, or damaged from prior childbirth, or in cases where the transvaginal procedure has failed, the surgery is performed transabdominally (through the abdomen). This article describes the outcome of 21 pregnancies following transabdominal cervical cerclage. Failure of previous transvaginal surgery and scarring of the cervix were the most common reasons for performing the cerclage by the transabdominal route. Prior to surgery, ultrasound was performed to determine the condition of the fetus. Surgery was performed during week 12 of pregnancy. Eighteen of the infants were carried to term and were normal at birth. There was one premature birth, with a favorable outcome, and there were two fetal deaths. After a review of the medical literature, it was found that 130 transabdominal cervical cerclage operations have been performed with a success rate of 89 percent. It is concluded that for a selected group of patients, those with a malformed or scarred cervix and for whom previous transvaginal cerclage attempts have failed, transabdominal cervical cerclage can be a safe and effective surgical approach that greatly improves the outcome of the pregnancy. Preconception transabdominal cervicoisthmic cerclage
Article Abstract:
The preconception transabdominal cervicoisthmic cerclage treatment for women with recurrent late pregnancy loss is discussed. This treatment resulted in a postoperative fetal survival rate of 100 percent for pregnancies that reached >12 weeks of gestation.
