Funding for patient-oriented research: critical strain on a fundamental linchpin
Article Abstract:
Patient-oriented research (POR) grant applications appear to have a lower priority with the National Institutes of Health than laboratory-oriented research (LOR) grant applications. Researchers analyzed the status and ultimate outcome of 12,000 grant applications reviewed by the NIH in 1994. Compared to LOR grants, fewer POR grants were ultimately funded and fewer reached the top 20th percentile in priority. POR grant applications that were reviewed by people who often reviewed POR grant applications were just as likely to be funded as LOR grant applications. This was not true if the application was reviewed by people who usually reviewed LOR applications.

Preventing the extinction of the clinical research ecosystem
Article Abstract:
A concerted effort at the national level is needed to ensure that funding is made available for clinical research and that physicians have the time and training to do it. Since the 1970's, physicians have noted the declining interest in research among young physicians. Federal funding for patient-oriented research has also declined. Many medical schools are losing revenue in their competition with managed care organizations and thus cannot finance clinical research. Representatives from medical schools, industry, government agencies and Congress need to work together to solve this problem.
