The investigation into the cause of the rail crash in Great Heck, England, in which 13 people died is discussed. It is possible that Gary Hart, the driver of the Land Rover which left the M62 and landed on the track, may have fallen asleep while driving.

Several factors contributed to the rail crash in Paddington, London, England, in Oct 1999 in which 31 people died, according to the official report into the incident. These included the fact that a red signal was difficult to see properly.
