The Chemical Safety and Hazard Investigation Board, which as been investigating the cause of a fatal explosion at BP PLC's Texas refinery, puts the blame on cost-cutting efforts by the company. Fifteen people were killed and 180 injured in the accident. BP is facing a criminal investigation and civil claims as a result of the explosion.
