Human errors in fatal and serious occupational accidents in Finland
Article Abstract:
Rasmussen's SRK model, developed for classifying human errors in complex nuclear power plants, is applicable to industrial units using simple technology. A study examining the role of human errors in industrial accidents shows that 84% to 94% of these accidents are due to human errors. The maximum errors are skill-based, followed by rule-based errors, and knowledge-based errors. The type of error is analyzed by the model in terms of age, work experience, and work tasks of the victims. An analytical classification of human errors may help prevent accidents.

Error analysis of ticket vending machines: comparing analytic nd empirical data
Article Abstract:
Three types of vending machines, namely the Few Fare Machine, the Multi Fare Machine and the overground machine, which are installed at London's underground and overground train stations, were subjected to a Dialogue Error Analysis (DEA) to determine the error-making patterns of the machines. Observations were made twice with 5 years interval. Comparison of the results show that initial inspection was already successful in predicting the both system-related and user-related patterns of error-making.

Equation error versus output error methods
Article Abstract:
The viability of human behaviour simulation techniques in terms of one-step-ahead predictivity and simulation capability were evaluated. The equation error criterion was primarily intended for short-horizon prediction while the output error criterion was mainly designed for infinite-horizon simulation. A simulation model validated the ineffectivity of equation error estimates for system modeling.
