The health care industry in Minneapolis, MN, has gained the reputation of being progressive and successful, but, in reality, the city's managed care system has developed some weaknesses. One test of the health of a managed care system is its rate of prepaid per-member/per-month arrangements with both hospitals and doctors, and the Twin Cities rate is among the lowest in comparison to other developed markets. In addition, the market is dominated by three health plans.

Has managed care lost its soul?

Managed care's original purpose of making health care affordable and accessible has shifted to a focus on controlling costs. Health maintenance organizations (HMOs) and other managed care plans have increasingly changed to for-profit status. An important factor has been the desire of employers to hold down health care expenses. Another factor was former Pres Ronald Reagan's move to end federal grants to HMOs, forcing them to seek financing from investors.

The fallacies of failure; Harvard's Lucian Leape says bad systems, not people, cause medical errors
Article Abstract:
Harvard researcher Lucian Leape, M.D., has determined that inadequate systems, not inept people, are the cause of most hospital errors. A system that decreases the chance of mistakes while increasing the chance of finding out about them before they harm a patient needs to be implemented. Standardizing procedures, computerizing drug prescriptions, and avoiding fatigue are strategies that can decrease errors.
