Many healthcare industry observers are bullish on the prospects for an international managed-care market. However, American health maintenance organizations who are ready to take the plunge to see if they can export their concepts should take a few precautions. They should carry out their projects with humbleness, as many countries often are already working more efficiently at lower cost than in the US, even while providing universal health coverage.

American Assn of Retired Persons Legislative Dir John Rother discusses his yearlong trip around the US investigating managed care plans. He found there was a lot of variability in the plans and the quality of the medical director was crucial to the viable operation of the managed care plan. Primary care was handled well in most plans but chronic care and post-acute care were not.
