US medical groups are setting up clinics in countries that lack access to high quality medical specialists. For example, Columbia-Presbyterian Medical Center runs a subsidiary in Moscow, Russia, and plans to open another in St Petersburg. Competition and cost cutting in the US influence efforts of medical groups to increase profits by exporting their services and acting as consultants abroad. Also, fewer foreign patients are expected to travel to the US for expert care.

Telephonic nurse counseling services are booming, and some observers believe that the telephone-based service will evolve into the point-of-entry for the healthcare system. Though counseling services typically are staffed by registered nurses that do not give medical advice, physicians are worried that the service may adversely affect their business. Managed care plans and capitation plans are big users of the counseling services since it reduces healthcare consumption.

Telemedicine: new tools, not new medicine

Telemedicine is expanding by the use of new high technology tools, such as video cameras, interactive software, cable and fiber optics, video compression and electronic scopes. Lower technology costs have further expanded the electronic extension of the healthcare industry. Intranets and the Internet are allowing the transmission of medical data to regional, national and even international areas.
