US medical groups are setting up clinics in countries that lack access to high quality medical specialists. For example, Columbia-Presbyterian Medical Center runs a subsidiary in Moscow, Russia, and plans to open another in St Petersburg. Competition and cost cutting in the US influence efforts of medical groups to increase profits by exporting their services and acting as consultants abroad. Also, fewer foreign patients are expected to travel to the US for expert care.

Managed care plans and capitation plans are big users of the counseling services since it reduces healthcare consumption.
