A randomized trial of N-acetylcysteine for prevention of trimethoprim-sulfamethoxazole hypersensitivity reactions in Pneumocystis carinii pneumonia prophylaxis (CTN 057)
Article Abstract:
N-acetylcysteine (NAC) does not appear to prevent adverse reactions to trimethoprim-sulfamethoxazole (TMP-SMX). TMP-SMX is used to prevent Pneumocystis carinii pneumonia in HIV patients. However, many patients are allergic to the drug. The allergic reaction is believed to be a response to a metabolite of the drug. Glutathione is a naturally occurring antioxidant in the body that detoxifies drug metabolites. If glutathione levels are low, it can be produced in the body by giving patients NAC. However, a study of 238 HIV patients randomized to receive NAC or a placebo before taking TMP-SMX found no difference in allergic reactions between the two groups.

Article Abstract:
The impact of switching treatment-experienced patients with virologic failure to a salvage regimen with or without a twelve week structured treatment interruption (STI) is studied. The proportion of patients with three months of sustained suppression of HIV does not increase with the STI regimen.

The role of adherence to antiretroviral therapy in the management of HIV infection
Article Abstract:
High level of therapeutic adherence is required for treatment success with highly antiretroviral therapy (HAART). The availability of once daily and coformulated agents with simple dosing requirements helps the patients to improve the adherence and there by make HAART a success.
