Adenomyosis in Pakistani women: four year experience at the Aga Khan University Medical Centre, Karachi

Adenomyosis is a relatively rare condition characterized by the invasive growth of endometrial tissue (the uterine lining) into the underlying muscular layer of the uterus. Much research is still needed to identify the cause and to develop effective treatments for this condition. Recently, an unexpectedly high number of cases of adenomyosis was retrospectively identified among Pakistani women who underwent a hysterectomy at the Aga Khan University Medical Centre in Karachi. These cases came to light during a quality assurance review conducted by the Medical Centre, covering cases between January 1986 and December 1989. A review of these cases is presented. Characteristics investigated were patient age, pregnancy history, and other diseases associated with the condition. Out of 419 specimens taken at hysterectomy, 237 (57 percent) met the criteria for adenomyosis. The patients ranged in age from 31 to 74 years old, with 196 (83 percent) being between the ages of 40 and 59. Most of the women (98 percent) had at least one child. Endometrial hyperplasia (uterine lining overgrowth) was the most common characteristic associated with adenomyosis. Fibroids (a type of tumor) and cervicitis (inflammation of the cervix) were also commonly seen. The underestimation of the prevalence of adenomyosis in this population is attributed to the fact that the only definitive means of diagnosing this condition is hysterectomy, which is also the only effective treatment. In the future, the identification of blood serum markers for adenomyosis may allow for presurgical diagnosis and better treatments for this condition. The travail of the university
Article Abstract:
Universities are in travail, under pressure to act like corporations, look at the bottom line, and ignore long-range effects of new ideas. Government support of scientific research is cut back. Students are increasingly ill-prepared. Such slogans as 'mathematics for all' are leading to watering down of math, especially calculus. For faculty, tenure is a concern, especially at state universities; academic freedom is thus threatened. There is distortion of the purpose of graduate study, which should be to prepare an original thesis. The university is supposed to be independent faculty and students eager for knowledge.

Helping trauma victims 'failed by system.' (Fife College and University of Abertay Dundee set up trauma management initiative)
Article Abstract:
Fife College and the University of Abertay Dundee have joined together to look at the needs of trauma victims. The initiative will involve research into trauma, taking into account new evidence which indicates that trauma victims can be seriously damaged by existing approaches to dealing with their problems. It will look at a number of key areas, including the development of diploma and degree courses in trauma management issues and the development of victim advocacy schemes.
