Allergic granulomatosis and angiitis (Churg-Strauss syndrome): response to 'pulse' intravenous cyclophosphamide
Article Abstract:
Churg-Strauss syndrome is characterized by allergic granulomatosis, the formation of many granular tumors, and by angiitis, the inflammation of the blood or lymph vessels. This syndrome is also associated with hypereosinophilia, a marked increase in the number of eosinophils (a type of white blood cell) and systemic vasculitis (widespread inflammation of blood vessels). This syndrome may occur in patients with asthma and allergic rhinitis (inflammation of the nasal mucosa). The infiltration (accumulation of substances in the lung) is symptomatic of the vasculitis phase and occurs in 72 percent of patients with Churg-Strauss syndrome. A case is described of a 51-year-old woman with Churg-Strauss syndrome who showed symptoms of vasculitis and lung infiltration. The patient rapidly deteriorated to respiratory failure, which was not alleviated by corticosteroid treatment. After the anticancer agent cyclophosphamide was given intravenously, the patient showed rapid clinical and biochemical improvement. Cyclophosphamide, which has the ability to suppress immunity, markedly decreased the number of eosinophils and other white blood cells which are involved in inflammatory and allergic responses. After being discharged, the patient was maintained on corticosteroids and antiasthmatic drugs. The findings show that intravenous cyclophosphamide is the treatment of choice for extensive vasculitis associated with Churg-Strauss syndrome. This article is designed to help clarify the possibility that the term "microscopic polyangiitis" or "microscopic polyarteritis" (MPA) may be misnomers, and the result of that can be improper definition of the disorder, resulting in a distorted view of its etiology and treatment. It contrasts those terms for a specific disorder against polyarteritis nodosa" as a way of highlighting the potential mishaps incorrect definition can bring.
