Physicians should run their practice like a business, managing their staffs to fulfill patients' health needs at the lowest cost while providing the best possible care. Physicians need to set realistic work standards for their staffs, communicate openly, constructively and effectively with them, and establish a fixed and fair salary schedule to obtain the best performance from their staffs.

Many observers remain perplexed at how little impact managed care has had on the Chicago, IL, healthcare market. Many hospitals in the market, in preparation for managed care, have linked with physicians and created large health systems. But HMO penetration, at just over 20%, remains low, and physicians generally have not formed large, integrated groups prepared to accept financial risk.
