The community: where reform rolls on
Article Abstract:
Communities are taking an active role in health care reform as evidenced by the formation of community care networks. The American Hospital Association suggests that an efficient, responsible health care system depends on action by community leaders, physicians, hospitals, and local government. The traditional medical emphasis on treating the injured and sick must be broadened with efforts to improve the health of the population. Models for health care delivery are shaped by market forces and the recognition that networks are an efficient method of delivering care when operating within a limited budget. Community care networks are successful because they are locally-based and focused. This health care model coordinates care and controls costs. To provide good medical care, these networks will have to merge creatively and aggressively with the community, cooperate with human services agencies, and function in partnership with physicians, hospitals, health systems, and other entities.

Direct-to-Consumer Marketing of Prescription Drugs: Creating Consumer Demand
Article Abstract:
The FDA should regulate direct-to-consumer (DTC) advertising by the pharmaceutical industry more strictly. The industry began marketing drugs directly to consumers in the 1980s, and spending on DTC advertising rose from $13.1 million in 1989 to more than $900 million in 1997. Although the industry claims the public's health will benefit, financial motives are clearly visible. The industry is creating consumer demand for drugs, which could damage the physician-patient relationship. Some doctors are more likely to prescribe a drug if the patient demands it, despite their better judgment.
