Article Abstract:
There are over 66 types of human papillomavirus (HPV) that are associated with genital warts, skin, cervical and genital cancer. The presence of (HPV) in tumors of sun-exposed skin but not in normal unexposed skin suggests that HPV and ultraviolet radiation are involved in nongenital skin malignancies. This virus is common in HIV infected people in benign and cancer-causing forms. As AIDS patients survive longer, HPV-induced invasive squamous cell cancer may become a major problem. People with familial dysplastic nevus syndrome have a high risk of melanoma. The greater concern is the 10% to 50% of the general population with the sporadic form who may also have a greater risk of melanoma. There is a high cure rate for melanoma detected early, but it is incurable after it has spread. Photochemotherapy has been used successfully in conditions involving the immune system. Systemic sclerosis, pemphigus vulgaris and mycosis fungoides have responded well to this treatment.

Pulsed CO2 lasers can effectively remove scars from acne and reduce the signs of photoaging. It does not produce as much skin damage as continuous-wave CO2 lasers. However, swelling and redness can last several months. Facelifts can eliminate sagging skin, but the lifted skin will still contain evidence of photoaging and wrinkles. Dermabrasion is effective in removing the signs of aging, but it is very messy. Chemical peels include trichloroacetic acid and phenol, but they may not be effective on severely damaged skin and may result in pigmentation disorders. CO2 laser resurfacing is relatively painless and the resulting smooth skin is probably due to collagen remodeling. Silicone gel sheets have been very effective in preventing scars after surgery and also in treating burn scars. Scars have also successfully been treated with the 585-nm pulsed dye laser and liquid nitrogen cryotherapy.

Calcipotriene, approved by the FDA, is shown to treat psoriasis safely and effectively. It can also treat pityriasis rubra pilaris, ichthyosis vulgaris, and X-linked ichthyosis. Topical corticosteroids can cause cutaneous atrophy and telangiectasia. Lasers can treat cutaneous pigmented skin lesions, using pulsed light for selective targeting. For tatoo removal, lasers are more effective on black professionally done tatoos. Multi-colored tatoos require several treatments with various laser wavelengths. New treatments for fungal infection of the nails include itraconazole and terbinafine, and are more effective than using griseofulvin.
