Eating, ethics and Alzheimer's
Article Abstract:
Alzheimer's disease can affect a patient's ability to swallow, which can lead to food refusal. Food refusal consequently raises an ethical dilemma of whether the patient should continue to be fed from a spoon or whether a nasogastric tube should be used. The tube is impersonal, invasive and it may not be desirable to preserve life in this way. Suggestions for improving care in such situations include: consulting relatives about the problem at an early stage; providing information; and having a nurse available to talk to. A related article on advice for carers is included.

Clinical view
Article Abstract:
A three-year ward-based training course for UK nurses, based on National Vocational Qualifications, would help solve the shortage of nurses. The new scheme would bring back training to the wards and give hospitals more staff at various levels, as in the past when nurses trained at local hospitals' Preliminary Training Schools.
