End-stage renal failure: the challenge to the nurse
Article Abstract:
End-stage renal failure (ESRF) is an incurable condition that can be managed through either long-term therapy treatment or kidney transplantation. The two main types of renal-replacement therapies are peritoneal dialysis or haemodialysis, both designed to control the effects of ESRF and both able to be adapted to hospital-based or home-based environments. Nurse-led self-care treatment programmes are not only cost-effective but give the clients more control over their treatment. The value of self-care programmes is assessed and the nurse's role is discussed.

Symptom relief in ESRF
Article Abstract:
End stage renal failure can be caused by long-term diabetes and needs expert nursing care. The nursing care of a 65-year-old woman who had been diabetic for 20 years and was admitted to hospital with end-stage renal failure is described. Major problems include confusion caused by chemical imbalances in her blood and water retention. Diet-control and blood pressure monitoring play an important part in her treatment after she leaves hospital.

The UK medical profession needs to make a greater effort to support unpaid carers, and the subject needs to be included in the syllabus of educational courses for nurses. The Carers Impact Project aims to assist commissioners and managers within the National Health Service to provide support and respite for carers. The Carers National Association has proposed a number of topics for inclusion in nursing courses.
