Efficacy and safety of intracavernosal alprostadil in men with erectile dysfunction
Article Abstract:
Injections of alprostadil into the base of the penis may be an effective treatment for impotence. Researchers used three groups of impotent men to determine the proper dose of alprostadil and how effective the drug would be. The duration of erection varied with the dose of the drug, ranging from 12 minutes following a 2.5 microgram dose to 44 minutes after a 20 microgram dose. In the final study of drug effectiveness, 471 men self-injected the drug at home over a period of six months. Seventy-eight percent of the men used a dose of 20 micrograms or less and 60% used a dose between 7.5 and 20 micrograms. Eighty-seven percent of the injections resulted in erections strong enough for satisfactory sexual intercourse. Half the men reported penile pain at some point, but not after every injection. Only 6% dropped out because of the pain. Six percent experienced possible complications of alprostadil treatment, including pain and swelling of the scrotum, pelvic pain and increased urinary frequency.

This review discusses the pathology and physiology of erectile dysfunction, its treatment, and patient management. Research shows that oral treatment has replaced invasive methods. Results indicate that because of erectile dysfunction's associaion with serious medical conditions, a multidisciplinary treatment approach will be more effective.
