Evidence-based practice in pressure sore reduction
Article Abstract:
Chase Farm Hospitals National Health Service Trust in Enfield, England, has undertaken a two-year programme to encourage evidence-based practice in pressure sore prevention. The project team was made up of members representing a range of disciplines, including nurse education and performance management. A number of challenges were encountered, including the need to move practice away from the use of pressure sore risk-assessment tools alone. However, the project resulted in patients at Chase Farm Hospital being at much reduced risk of developing a pressure sore.

Making sense of pressure sore prediction calculators
Article Abstract:
Pressure sore calculators facilitate early preventive interventions for patients most at risk. The Norton scale and Pressure Sore Prediction Score assess patients patients according to physical and mental fitness, mobility and incontinence. The Waterlow risk assessment card is more comprehensive establishing degrees of risk. The Douglas pressure score calculator extends the scope of the Norton scale considering pain and nutritional factors.

Improving policy and practice in the prevention of pressure ulcers
Article Abstract:
A project to update policy and practice in pressure-ulcer prevention in a district general hospital is reported. The training of staff and the educational needs of staff within the trust are analyzed and the literature reviewed for the purpose covered the evidence-based tools for risk assessment, treatment protocols, policy and patient information.
