General internal medicine and general internists: recognizing a national need
Article Abstract:
The Federated Council for Internal Medicine (FCIM) has developed a set of recommendations to balance the number of physicians in internal medicine who specialize with the number who practice general medicine. There is a shortage of general internists in the US, and the shortage could worsen by the year 2000. Fewer medical school graduates are choosing internal medicine as a specialty, and most residents in internal medicine are not choosing general medicine. The FCIM recommends that half the graduates from internal medicine residency programs practice general medicine. Medical schools should provide the best quality of education in internal medicine and primary care medicine. Hospitals and the government should join together to make internal medicine an attractive career option. Efforts should be made to make office practice more economically feasible and the federal government should provide more money for research and education in general medicine.

Article Abstract:
The proliferation of subspecialties in internal medicine is detrimental to the field. Increasing specialization in internal medicine will lead to more competition for funding, more difficulty in conducting research and more difficulty in training new physicians. Departments of medicine that at one time housed all internal medicine subspecialties allowed for a freer exchange of information and economic resources. Internists need the broad training that a department of medicine can provide. It is the foundation upon which the subspecialties rest. As reimbursement for medical procedures is reduced by the introduction of more managed care health plans, it will be necessary for physicians to find the most cost-effective ways of providing treatment. This will involve coordination between general practitioners and subspecialists.

General internists and family physicians: partners in geriatric medicine?
Article Abstract:
Family doctors and internists can treat elderly patients but they must change the nature of their practice to do so. Many elderly patients have chronic diseases that are poorly managed under the current US health care system. Medicare is also reducing the reimbursement rate it pays doctors for treating elderly people.
