Socioeconomic status and risk for substandard medical care
Article Abstract:
Patients without health insurance may be more likely to suffer medical injuries caused by substandard medical care than those who are insured. A survey examined the medical records of 30,195 patients treated in 51 hospitals in New York state in 1984. Among the 1,278 medical injuries that occurred among these patients, 306 were caused by substandard medical care. Patients who were uninsured had a significantly higher risk of receiving substandard medical care than those who were insured. More medical injuries caused by negligence occurred among black patients and poor patients than among other patients. The risk of receiving substandard medical care was still not significantly higher for black patients or poor patients than for other patients. The characteristics of the different hospitals at which the patients were treated did not determine the risk of receiving substandard medical care.

Relationship between malpractice claims and cesarean delivery
Article Abstract:
Malpractice claims do appear to increase the rate of cesarean delivery. In the US, rates of cesarean deliveries have jumped from 4.5 per 100 births in 1964 to 24.1 per 100 births in 1986. There has been much speculation among both the medical community and the public as to whether fear of malpractice claims has contributed to this increase in cesarean deliveries. In a state-wide study of 31 New York hospitals and 60,490 deliveries, delivery by cesarean section was noticeably related to physician malpractice insurance premiums, the number of claims against obstetric staff per 100 physicians and the number of claims against a hospital per 1,000 patients discharged. Also, how physicians perceived the risk of a malpractice suit was strongly associated with the rate of cesarean delivery.
