The practical steps that health and social care professionals can take to improve discharge processes in their care settings are presented. A related document, 'Achieving Timely Simple Discharge from Hospital: A Toolkit for the Multidisciplinary Team', released in the year 2004 revealed that 80% of discharges from hospital could be classified as 'simple' and could be 'nurse-initiated'.
