Bridging the care continuum for open heart surgery patients
Article Abstract:
A nursing program successfully spanned the continuum of care for recovering open heart surgery patients. In this pilot program, nurses made house calls two days after patients were sent home, and sometimes a second house call was made. Patient satisfaction and cost savings both increased, including costs per case and costs for postoperative length of stay. Permanent integration of the program into the hospital is described.

The Order of Saint Francis, Saint Francis Medical Center, Peoria, Il, rectified a challenging staffing problem without having to rely on the help of a nursing agency. The nursing administration created the "crisis nurse" nursing position to help overwhelmed staff in the Intensive Care Unit and Emergency Department.
