Article Abstract:
Home carers are being called upon to undertake nursing tasks which they are not qualified to do, leading to concerns that the quality of UK community nursing care is being cut back. An unpublished survey of 1,200 home carers revealed that 33% gave drugs to patients and almost one-fifth carried out tasks which they believed should be performed by nurses. These tasks included, changing colostomy bags, catheter bags and dressings, urine testing and giving enemas. Home carers are concerned over insurance and there are also fears that patients' lives may be at risk.

Case management is an effective method of dealing with all the problems of clients with learning disabilities. The case manager handles all parts of the care of the client. After the client's needs have been assessed by field workers and a plan of action drawn up, the case manager will provide a package of resources which will allow the client to stay within the community, rather than enter a residential home. Information on two case histories is included.

Managing care at home
Article Abstract:
Pilot schemes assessing community mental health service provision have established the need for a multi-disciplinary approach coordinated by a care manager. The care manager assesses needs, negotiates care with health and social services and monitors success. Managers may assume a therapeutic role in establishing relationships with clients. Budget control facilitates the establishment of a care package.
