Influence of race, sex, and age on management of unstable angina and non-Q-wave myocardial infarction: the TIMI III Registry

It may be important to treat women and the elderly more aggressively for episodes of unstable angina. Unstable angina is chest pain that occurs when the heart does not receive enough blood, a condition called ischemia. Researchers used data from the Thrombolysis in Myocardial Ischemia (TIMI) trial to determine whether different demographic groups receive different treatment for unstable angina and whether their outcomes are different. They followed 3,318 patients admitted to 18 hospitals for unstable angina. Forty-nine percent were women and 28% were black. A total of 828 patients were older than 75. Blacks were less likely to receive aggressive treatment than whites, but coronary angiography revealed their coronary artery disease was less severe and they were less likely to have subsequent angina. Women were also treated less aggressively and even though they had less severe disease, their outcome was similar to that in men. Elderly patients were treated less aggressively even though they had more severe disease and worse outcomes than younger patients.
