Article Abstract:
The Hospital Medicines Management Collaborative (HMMC) has given the acute trusts the opportunity to work collaboratively with their local PCTs and other trusts inside and outside the HMMC to improve medicine management systems, since 2004. Healthcare professionals need to appreciate the roles of the pharmacy staff and the systems they work within, and working together with patients by forming partnerships in all aspects of prescribing and medicines management is the only way forward.
