Mind matters
Article Abstract:
A project was set up to help primary care staff recognize people with mental health problems. The project looked at developing care protocols because nurses sometimes feel they are acting outside their area of expertise. Closer links with primary health teams and psychiatric teams were established by appointing a link worker to liaise between the two teams. Training packages were introduced and their effectiveness evaluated afterwards. Voluntary organisations and ethnic groups were invited to talk to the teams to give their views. Other initiatives are still underway.

The mental health system in the US is dominated by insurance companies which insist on particular types of care, specify how long it is to be given and even what health staff are to be paid. Doctors are angry that medical decisions are being based on cost and not need. They are forced to use short-term measures which are not suitable for people with chronic mental illness and it undermines doctors' professional judgement. However, the insurers argue that doctors dislike having their fees reduced. These problems may begin to happen in the United Kingdom.

Self-management in primary care
Article Abstract:
Multidisciplinary primary health care teams can play a vital role in coordinating the wide range of work now undertaken in primary health care in the UK. These teams have a range of advantages, including enhanced creativity, raised ability to manage workloads and continuity and coordination of care. Working in a primary health care team can bring a great deal of knowledge and experience, with team members being able to develop their own ideas and feel more confident about expressing their views.
